The PRN Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert who provides ethical, evidence-based, safe, and compassionate nursing care. The PRN Ambulatory Clinic RN Care Coordinator focuses on patient safety and the quality of nursing care by identifying and clarifying patient needs, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes. This position specifically supports the Heart Failure Device Team and is responsible for coordinating the care of patients with advanced heart failure and implantable monitoring and therapeutic devices. The RN Care Coordinator manages patients with pulmonary artery pressure monitoring devices, including ongoing review of transmitted data, communication of clinical findings, patient education, and coordination of interventions in collaboration with the provider team. The RN Care Coordinator also participates in the evaluation and workup of patients being considered for implantable heart failure therapies and devices, coordinating testing, specialty consultations, patient education, and follow-up throughout the treatment journey. The RN Care Coordinator provides support to assigned physicians by communicating with patients, families, and caregivers and relaying pertinent information to the physician. They provide support during physician clinic sessions and ensure the patient, family, and caregiver understand the plan of care. The RN Care Coordinator serves as a key member of the multidisciplinary Heart Failure Device Team, promoting continuity of care, optimizing patient outcomes, and ensuring timely coordination of care across the ambulatory setting.
