About The Position
The Virtual Care Coordinator RN delivers telehealth hospice care, helping patients and families receive personalized support. The role uses virtual platforms to monitor patient conditions, coordinate care plans, and provide real-time guidance to field staff and caregivers. The position is part time, with a scheduled shift of 2:00 p.m. to 12:00 a.m. EST and a requirement to work at least 30 hours within a six-week period, based on business needs. The RN conducts virtual assessments and regular check-ins, coordinates individualized care with an interdisciplinary team, provides education and support on symptom management and end-of-life care, and responds to urgent virtual consultations. Trinity Health At Home is a national, faith-based, not-for-profit home care, hospice, and palliative care organization serving communities across 11 states.
Requirements
- At least 5 years of Home Care, Hospice, ER, and/or Triage RN experience.
- Active RN license in a Compact State (eNLC).
- BSN from an approved nursing education program.
- Proficiency in Epic or home care EMR experience.
- Strong communication and interpersonal skills to build rapport with patients and families virtually.
- Ability to work independently and as part of a collaborative remote team.
- Ability to adapt to rapid changes to meet business or department needs.
- Patient or customer service experience.
- Flexibility to work a minimum of 30 hours within a six-week period, based on business needs.
Responsibilities
- Conduct virtual assessments and remotely monitor patients' conditions.
- Develop and coordinate individualized care plans in collaboration with the interdisciplinary team.
- Provide virtual education and support to patients, families, and caregivers regarding symptom management and end-of-life care.
- Liaise with field nurses, physicians, social workers, and other team members to ensure comprehensive care delivery.
- Respond promptly to urgent virtual consultations and triage patient needs effectively.
- Use telehealth technology to conduct regular check-ins and follow-ups with patients and families.
