About The Position

The Triage RN/Care Coordinator provides clinical oversight, triage, and care coordination to hospice patients and families during non-business hours. This position ensures timely and appropriate interventions through a combination of telephone triage and in-person visits for urgent needs, new admissions and death pronouncements. The RN serves as the clinical lead after hours, making independent decisions, supporting caregivers in crisis and maintaining continuity of care.

Requirements

  • Current Registered Nurse license by the state at which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • Valid driver’s license.
  • Basic Life Support certification within 30 days of hire date.
  • Three (3) years’ experience as a Registered Nurse.
  • One (1) year of experience providing direct patient care in home care/hospice setting.
  • Strong critical assessment and critical thinking skills.
  • Ability to work independently in a fast paced, emotionally sensitive environment.
  • Strong and effective interpersonal and problem-solving skills utilized with all stakeholders.

Responsibilities

  • Respond promptly to after-hours calls from patients, families, facilities and caregivers regarding symptom management, medication concerns, changes in condition, or end-of-life support needs.
  • Use clinical judgement to independently triage calls and determine appropriate interventions, including whether in person visits are required.
  • Conduct timely in person visits for symptom management, emergent care needs, new hospice admissions and death pronouncements.
  • Perform comprehensive nursing assessments during in person visits and implement interventions aligned with the patient’s individualized plan of care.
  • Initiate hospice services during new admissions including completing required documentation, reviewing consents, educating caregivers on the hospice philosophy and setting initial care priorities.
  • Collaborate with the admitting physician, primary nurse, and interdisciplinary team (IDT) to ensure seamless care, coordination and continuity.
  • Accurately document all clinical encounters (phone and in-person) in the electronic medical record (EMR) per agency and regulatory guidelines.
  • Provide concise handoff reports to the primary hospice team to support continuity of care during business hours.
  • Communicate any significant changes in patient condition, medication needs or caregiver concerns to appropriate team members and physicians in a timely manner.
  • Support family members and caregivers during the time of death, including completing all death visit protocols and facilitating contact with funeral homes, physicians and other necessary parties.
  • Adhere to the hospice policies, state/federal regulations, and standards of nursing practice, always maintaining patient confident and professional conduct.
  • Participate in quality improvement initiatives, ongoing training and team meetings as required.
