About The Position

Requirements

• Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia.
• At least one year of hospice experience is required.
• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable car available for use during work hours.

Responsibilities

• Completing an initial assessment of the patient and family to determine hospice needs.
• Developing an individualized plan of care focused on comfort and quality of life.
• Managing and coordinating care with the interdisciplinary team.
• Supervising LPNs and hospice aides.
• Providing hands-on support for pain and symptom management.
• Spending meaningful time with patients and families.
• Monitoring, documenting, and communicating changes in condition.
• Obtaining and carrying out physician orders.
• Adjusting the plan of care as needed.
• Participating in on-call rotation per policy.
• Performing other duties as assigned.
