About The Position

The RN Case Manager coordinates patient care, supports utilization review, and ensures effective discharge planning in an acute care hospital setting.

Requirements

  • Active California RN License – Required
  • Minimum 2 years acute care Case Management experience
  • Experience in care coordination & discharge planning – Required
  • EPIC EMR experience – Required
  • Strong clinical judgment and communication skills
  • Knowledge of InterQual, Medicare, and insurance processes

Responsibilities

  • Conduct patient assessments and care coordination
  • Perform utilization review using InterQual criteria
  • Develop and manage discharge planning processes
  • Collaborate with interdisciplinary teams
  • Coordinate DME, SNF placement, and transport needs
  • Participate in patient and family care discussions
  • Ensure compliance with CMS, HIPAA, and regulatory standards
  • Document all activities in EPIC EMR
