About The Position

The primary function of this full-time LVN Intake Coordinator position is to facilitate the intake referral process and coordinate care with the interdisciplinary team and the referral source for Harbor Home Health of Houston.

Requirements

  • Graduate of an accredited school of vocational nursing
  • Current Texas State license as a Licensed Vocational Nurse
  • Current Texas Drivers License
  • One year experience as a Licensed Vocational Nurse
  • Nursing skills as defined as generally accepted standards of practice
  • Good interpersonal skills
  • Proof of Hepatitis profile
  • Experience with the intake process and procedures
  • Experience in verifying insurance, Medicaid and Medicare
  • Experience with Microsoft Office (Excel, Word) and Microsoft Outlook
  • Detail oriented and highly organized
  • Effective written and verbal communication
  • Clean background and drug screen
  • Reliable transportation

Nice To Haves

  • Home health experience preferred
