About The Position

Requirements

  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
  • One year of experience under license.
  • Great clinical and communication skills.
  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Can work independently and on your own on a day-to-day basis with activities that include: phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
  • Time management skills to manage visits, documentation, and meetings (i.e.: schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns).
  • Ability to learn an electronic medical records system.
  • Must be willing to drive with reliable transportation, a valid driver’s license, and auto insurance, to be able to travel across the service area.

Nice To Haves

  • Admissions and On-call experience preferred.
  • Work closely with patients on a 1-1 level but get strong support from a patient care team.

Responsibilities

  • Providing care and services for a group of hospice patients.
  • Visiting patients where they reside, which could include private homes, assisted living facilities, and hospitals.
  • Completing hospice admissions and patient visits as needed.
  • Answering calls from patients, families and customers.
  • Making nursing visits for symptom management, deaths, and admissions as needed.
  • Being a passionate advocate for both hospice services and Suncrest Hospice.
