About The Position

The Hospice Licensed Practical Nurse (LPN) provides direct, skilled nursing services to hospice patients in accordance with the Plan of Care and under the direction of the RN Case Manager. The role focuses on helping patients spend quality time with loved ones and take part in activities they enjoy, while building compassionate relationships with patients and families and educating caregivers on providing care. The LPN collaborates with the care team to promote comfort, peace, and dignity. The posting describes the schedule as five days per week.

Requirements
