About The Position
The Nurse Home Visitor (NHV) serves the Nurse-Family Partnership (NFP) program at the St. Mary's County Health Department, delivering comprehensive, evidence-based nursing services to first-time mothers and their families. Services prioritize women facing social, economic, and health-related vulnerabilities, including those disconnected from care or at increased risk for adverse maternal and child health outcomes. The NHV provides individualized, home-based care to improve maternal and child health, strengthen parent-child relationships, and support long-term family self-sufficiency. The role includes early screening, identification, and referral for substance use disorders—particularly opioid use disorder—and co-occurring mental health conditions. The NHV connects families to behavioral health services and collaborates with multidisciplinary teams to provide coordinated, trauma-informed care. The position also provides education and support to families impacted by Neonatal Abstinence Syndrome (NAS), including parenting guidance and practical strategies for caregivers and infants affected by in utero substance exposure. The NHV follows the Nurse-Family Partnership model and its policies, procedures, Visit-to-Visit Guidelines, and quality benchmarks. The position reports to the NFP Nurse Supervisor and partners with community agencies and Health Department programs to address complex needs and improve outcomes for high-risk families.
Requirements
- Bachelor’s degree in nursing from an accredited college or university, or an accepted education/experience substitution.
- Current Registered Nurse license from the Maryland State Board of Nursing.
- Provide the facility with a telephone number at which the employee can be reached.
- A valid Maryland motor vehicle operator’s license is required if assigned duties requiring operation of a motor vehicle.
Responsibilities

- Support improved maternal and child health, stronger parent-child relationships, and long-term family self-sufficiency.

- Adhere to the Nurse-Family Partnership model, policies, procedures, Visit-to-Visit Guidelines, and quality benchmarks.
