About The Position

The Heart Transplant Coordinator is responsible for managing the care of Heart transplant recipients throughout the post-surgical phase. This role involves patient education, coordination of care with a multidisciplinary transplant team and ensuring compliance with regulatory and transplant program guidelines.

Requirements

  • Graduate from a nursing program
  • 2 years of professional clinical experience
  • RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure
  • ACLS
  • BCLS - Basic Life Support

Nice To Haves

  • Cardiovascular focus experience
  • 1 year experience caring for LVAD patients and heart transplant patients

Responsibilities

  • Participates in the development, implementation, and maintenance of the left ventricular assist device (LVAD) program and the heart transplant program; to coordinate and manage the patient care of this high-risk patient population throughout the continuum.
