About The Position

The Director of Nursing leads and oversees nursing services for individuals with disabilities, focusing on person-centered care, regulatory compliance, and managing a dedicated team of community living arrangement (CLA) nurses who are responsible for providing high-quality care to residents with Intellectual and Developmental Disabilities (IDD). This pivotal role encompasses the management of nursing staff and ensures compliance with best practices and regulatory standards while fostering a culture of respect, trust, and growth within the team. The Director collaborates with healthcare professionals and stakeholders to develop effective care strategies to promote independence and enhance the well-being of all residents. The Director oversees medical preferred partner relationships. A commitment to continuous improvement and professional development is essential, as this role empowers nursing staff and upholds the dignity and needs of all residents. The Director of Nursing is in consistent communication/collaboration with the Chief Nursing Officer (CNO). The Director of Nursing takes a holistic approach in providing high-quality nursing care to all residents in the CLA.

Requirements

  • Bachelor of Science in Nursing (BSN)
  • Licensed as a registered nurse (RN) in the Commonwealth of Pennsylvania.
  • Three (3) years demonstrated experience with increasingly progressive management responsibilities in healthcare operations.
  • Three (3) years demonstrated experience in the supervision of licensed professional nurses in areas including, but not limited to, recruitment, selection, leadership, coaching, training, performance management, promotion, discipline, and termination
  • Two (2) years’ clinical experience working with individuals living with multiple disabilities, including a strong understanding of the safety and communication support required for individuals with physical disabilities.
  • Thorough knowledge of Pennsylvania Chapter 6400 regulations.
  • Knowledge of HRST processes and requirements.
  • Strong understanding of medication administration, documentation standards, and healthcare regulations.
  • Demonstrated ability to work in a fast-paced environment and effectively prioritize tasks.
  • Demonstrated strong attention to detail.
  • Demonstrated ability to work effectively as part of a team.
  • Demonstrated strong time management and organizational skills.
  • Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision.

  • Excellent customer interaction, collaboration, and written and verbal communication skills.
  • Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, SharePoint, and PowerPoint.
  • Experience using electronic management/electronic records systems.
  • Must be able to perform CPR, First Aid, and crisis intervention using agency-trained protocols.
  • Must possess a current, valid driver's license in the state of residence, have a minimum of three (3) years of driving experience within the United States, and have an acceptable driving record, which will be assessed through a review of the Division of Motor Vehicles (DMV) record.
  • Available for evening and weekend work as needed and scheduled.
  • Ability to travel throughout three (3) counties – Delaware, Chester, and Bucks – as needed.

Responsibilities

  • Provide supervision of all assigned nurses in the areas including, but not limited to, recruitment, selection, leadership, coaching, training, performance management, promotion, discipline, and termination.
  • Provide supervision of and guidance to the Clinical Education and Compliance Nurse.
  • Develop and implement individualized healthcare plans, oversee quality and manage complex individual needs.
  • Ensure adherence to local, state, and federal regulations, accreditation standards, and documentation requirements related to nursing in IDD.
  • As a bridge between staff, administrators, medical providers, families, and regulatory bodies, ensure that communication regarding individuals and their care needs/status is timely and clear.
  • Contribute to program development, policy creation, and strategic vision for clinical services.
  • Actively participates and contributes to the Clinical Governance Meeting.
  • Conduct monthly audits of nursing activities including, but not limited to, monthly weights, monthly head-to-toe assessments, monthly medication audits, weekly CLA meeting minutes, monthly medication reconciliation, Medication administration record audits and eMAR audits, and reviews hospital discharge paperwork to ensure follow ups occur.
  • Create and deliver onboarding, training, assessment, and evaluation plans for new CLA nurses to ensure consistent and thorough knowledge of CADES processes, procedures, and care plans for individuals.
  • Communicate and coordinate health information between CADES, Program Directors, outside provider agencies, and other health professionals to ensure continuity of care routinely and whenever further assessment, evaluation, or monitoring is required.
  • Screen individuals for illness/injury and determine the need for the implementation of routine, urgent, and emergency care as identified in CADES nursing policies and procedures.
  • Prepare and administer medications, treatments, and tasks as ordered and in accordance with CADES policies and procedures and Pennsylvania 6400 regulations as necessary.
  • Evaluate therapeutic effectiveness of medication with pertinent data and report any observed side effects or relevant laboratory values to the appropriate physician.
  • Monitor and oversee medication administration/issues for individuals to ensure accurate administration, documentation, and storage.
  • Lead meetings that pertain to key performance indicators and quality care metrics.
  • Oversee CLA nursing meetings, in-service training, and other meetings as necessary and ensure meeting minutes are accessible to all nursing staff.
  • Review all hospital discharge reports, ensure appropriate follow-up actions are completed in alignment with all recommendations, and provide feedback to primary care physician on recommendations from discharge summaries for follow-up.
  • Maintain health care records in alignment with PA Department of Human Services regulations (Chapter 6400) and Medicaid Waiver regulations (Chapter 51).
  • Collaborate with the Chief Nursing Officer to develop and implement client annual assessments, health screening tools (such as HRST), Individual Service Plans (ISP), health plans, and Plans of Care; update electronic health record(s) for any change in status throughout the ISP year.
  • Assist with disposable Durable Medical Equipment (DME) and supplements for clients on caseload in collaboration with residential staff as needed.
  • Audit CLA electronic health record documentation as directed.
  • Implement new clinical initiatives as directed by the Chief Nursing Officer.
  • Inventory stock and confirm ordering to ensure each home has an appropriate level of medication and medical equipment/items.
  • Communicate with client’s family members and designated health care decision makers to ensure medical-related decisions are made as a team.
  • Read daily clinical reports and provide feedback as needed.
  • Read and review electronic health records daily and provide feedback, acknowledgements, and act as necessary.
  • Provide expert opinions and training in the field to ensure the best possible care of individuals.
  • Perform other duties as assigned
