About The Position
UVA Health is seeking a compassionate, motivated Clinical Social Worker to join its Dialysis team in Staunton. The social worker advocates for and supports patients receiving life-sustaining dialysis treatment, helping them navigate transportation, financial challenges, insurance concerns, care planning, and other barriers to successful treatment. The role builds ongoing relationships with patients and works as part of an experienced, supportive interdisciplinary healthcare team serving rural and urban communities. The position coordinates psychosocial aspects of patient care, conducts required assessments and depression screenings, supports insurance verification and financial assistance, and helps patients complete income evaluations and required paperwork. It also assists with transitions to nursing homes or other care settings, maintains timely and accurate social work documentation and care plans, and connects patients and families with community resources. The social worker advocates for patients, participates in the interdisciplinary care team, and identifies ways to improve patient support. The role may also involve transplant-related support activities. A master's degree in Social Work from a CSWE-accredited program is required. No relevant experience is required; healthcare and medical social work, dialysis, renal, chronic disease management, and work with vulnerable or medically complex populations are preferred. The employee is expected to meet UVA Health's eligibility requirements for employment as a Clinical Social Worker and to become licensed according to departmental guidelines.
Requirements
- Master's degree in Social Work from a CSWE-accredited social work program.
- Eligibility for employment as a Clinical Social Worker according to UVA Health requirements.
- Expectation to become a Licensed Clinical Social Worker according to departmental guidelines.
- Strong organizational, communication, and time-management skills.
