About The Position

Requirements

Master's degree in Social Work from an accredited program.
LLMSW required; must obtain full licensure after completing 4,000 supervised hours within 12 months.
Ability to interview, assess, organize, and problem-solve.
Familiarity with PCs and ability to analyze data.
Exposure to or experience in pre-acute and post-acute care, as well as community resources.
Ability to work independently and collaboratively with diverse teams and community agencies.
