About The Position

ChesPenn is a network of community health centers in Delaware County dedicated to providing quality, integrated, and culturally sensitive medical, dental, and behavioral healthcare to all individuals, regardless of their financial status. The position focuses on designing, promoting, implementing, and evaluating comprehensive health education programs for at-risk populations to improve health outcomes. Additionally, it includes assessing patients' medical and social needs and connecting them with appropriate services.

Requirements

  • Master’s Degree preferred
  • At least two (2) years as a registered/licensed Dietitian
  • Experience providing education to people among various ethnic and social backgrounds
  • Experience in an outpatient practice setting preferred
  • Excellent organizational skills to manage numerous practice activities
  • Excellent interpersonal skills, team player, professionalism, responsible, courteous and reliable
  • Proficiency in computer skills, familiarity with Electronic Medical Records and willingness to learn all computer systems.
  • Demonstrates initiative, leadership and a high level of educator skills
  • Unconditional ability to maintain HIPAA regulations
  • Demonstrates cultural competency among persons from various ethnic and social backgrounds
  • Leadership skills that include the ability and desire to champion teambuilding within primary Care.
  • Demonstrates fiscal responsibility
  • Demonstrates effective time management skills and follow through

Responsibilities

  • Work collaboratively with primary care providers to identify at-risk patients needing health education and to address broader population needs in health promotion and disease prevention, including specific conditions like diabetes, weight management, cardiovascular disease, hypertension, renal disease, malnutrition, failure to thrive in infants, cancer nutrition therapy, gastrointestinal disorders, and sports nutrition.
  • Design and implement high-quality group education programs for at-risk patients and their families to enhance positive health outcomes and reduce high-risk behaviors.
  • Continuously evaluate the content of group sessions to ensure high quality and adherence to standards of care.
  • Collect and analyze measurable outcomes and data.
  • Provide patient education, counseling, and assistance in obtaining necessary services and supplies to patients of all ages.
  • Offer comprehensive, individualized health education and self-management support to patients in coordination with primary care providers.
  • Advance the development, implementation, and evaluation of health education programs.
  • Participate in health education and promotion activities in collaboration with the Community Health Educator, Director of Clinical Support, Director of Patient and Community Services, and Chief Medical Officer.
  • Maintain and submit monthly reports to the Chief Medical Officer detailing encounters, group sessions, and diabetic education activities.
  • Serve as a liaison and representative at meetings, workshops, and conferences as required.
  • Develop appropriate health education materials and programs for use within the health center, in conjunction with the Chief Medical Officer.
  • Attend and participate in meetings, in-services, educational programs, and other relevant activities.
  • Adhere to current Health Center policies and guidelines.
  • Perform other assigned duties and responsibilities.
