About The Position

The Clinical Coordinator is an LPN or RN who serves as nursing support for the interdisciplinary staff, office staff and external customers. The Clinical Coordinator has a deep understanding of field staff duties, enabling the coordinator to offer resources and respond effectively to those seeking assistance from the office.

Requirements

  • Excellent verbal and written communication skills.
  • Excellent interpersonal, negotiation, and conflict resolution skills.
  • Excellent organizational skills and attention to detail.
  • Ability to remain calm and troubleshoot/handle patient/family crisis situations.
  • Ability to prioritize tasks and to delegate them when appropriate.
  • Ability to act with integrity, professionalism, and confidentiality.
  • Familiar with EMR systems.
  • Must have a valid Florida Driver’s license, reliable transportation, and current automobile insurance.
  • Valid/current Florida and/or Multistate compact licensure as an RN.
  • 3 years of nursing experience, preferably in hospice or home health.
  • Hospice/Palliative Care experience required.
  • Valid State of FL or Compact State RN License

Nice To Haves

  • Bachelor of Nursing Degree preferred.
  • CHPN preferred.

Responsibilities

  • Provides clinical resources to oversee patient and family inquiries and assess patient needs.
  • Assists w/receiving and triaging patient related phone calls during office hours.
  • Provides support to case managers by ordering medications, responding to facility questions, collaborating with 24-Hour Care Coordinator for DME and supply needs, etc. and communicating changes to appropriate team members.
  • Responds to patient needs in a timely manner based on medical priority.
  • Documents all incoming calls in the electronic medical record and notifies appropriate team members, providing documentation of such notification as indicated, ensuring compliance with BBH policy and procedure.
  • Manages and documents patient phone calls pertaining to medical conditions and questions according to established department procedures.
  • Serves as liaison between physicians, nurses, and patients.
  • Navigates the collaboration between physicians, nurses, and patients to resolve questions concerning medications, health education, and medical advice within skill level and scope of practice.
  • Records and maintains complete and accurate records of all communication between BBH, patient and physician regarding medical care in accordance with BBH medical record documentation requirements.
  • Responds to patient needs in an appropriate manner within skill level and BBH policies and, when appropriate, seeks provider advice when responding to patient’s medical care.
  • Utilizes relevant data, such as medical records and patient complaints, observations, and listening skills, to identify and resolve problems in nursing care.
  • Reviews clinician documentation.
  • Conducts chart audits on a regular basis, and communicates findings to Director of 24-Hour Care
  • Collaborates with evening triage to ensure shift reports are given on any pertinent information impacting patients, the 24-hour teams, or BBH.
  • Collaborates with Admissions teams to schedule any necessary 24-Hour admissions, or follow-up.
  • Schedules team follow-up visits or calls as needed per request from IDG.
  • Reviews hospice plans of care.
