About The Position

The primary purpose of your job position is to provide each of your assigned residents with routine daily care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves ensuring residents are comfortable, assisting with daily living activities, maintaining their living space, and accurately documenting care provided. The caregiver is expected to create a warm and positive environment, respond promptly to resident needs, and adhere to all facility policies and procedures. This position also requires maintaining resident confidentiality and reporting any concerns or incidents promptly to supervisors.

Requirements

  • High school education or its equivalent.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Proficient in Microsoft Suite products.

Responsibilities

  • Provide routine daily care and services to assigned residents according to their assessment and care plan.
  • Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
  • Report all changes in residents' conditions to the Health and Wellness Director promptly.
  • Record all entries on notes, SPAs, and incident reports in an informative and descriptive manner.
  • Report all observed accidents and incidents to the Executive Director and Health and Wellness Director on the shift they occur.
  • Maintain resident confidentiality and promptly report suspected or known violations.
  • Report any known or suspected unauthorized attempt to access the community's information system.
  • Perform only trained care tasks.
  • Ensure resident rooms are ready, including making beds.
  • Greet residents, escort them to rooms, and introduce them to others.
  • Make residents comfortable.
  • Inventory and mark residents' personal possessions and store clothing.
  • Assist residents with packing personal possessions for transfers or discharges.
  • Transport residents to new rooms or receiving areas.
  • Assist with loading/unloading residents to/from vehicles.
  • Create and maintain a warm, positive, and calm environment.
  • Meet with nursing personnel to identify and correct problem areas.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds (occupied and unoccupied) and put extra covers on beds as requested.
  • Ensure residents unable to call for help are checked frequently.
  • Answer resident calls promptly.
  • Check residents routinely to ensure personal care needs are met.
  • Assist residents with identifying food arrangements.
  • Keep residents' water pitchers clean and filled with fresh water within easy reach.
  • Perform after-meal care (remove trays, clean hands, face, clothing).
  • Receive the nursing report upon reporting for duty.
  • Perform all assigned tasks in accordance with established policies and procedures.
  • Follow work assignments and schedules.
  • Cooperate with inter-departmental and facility personnel.
  • Notify the facility when late or absent.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions (dental/mouth care, bath, hair combing, dressing/undressing).
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist residents in preparing for medical tests.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing residents for physical examinations.
  • Check each resident routinely to ensure personal care needs are met according to their wishes.
  • Observe and report the presence of pressure areas and skin breakdowns.
  • Report injuries of unknown source, including skin tears.
  • Observe disoriented and comatose residents.
  • Record and report data as instructed.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse of any resident leaving/missing from the facility.
  • Use only trained equipment.
  • Report defective equipment to the Nurse Supervisor/Charge Nurse.
  • Maintain confidentiality of all resident care information.
  • Report known or suspected incidents of unauthorized disclosure of information.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident's refusal of treatment requests and report such requests to a supervisor.
  • Give back rubs as instructed.
  • Assist residents with hair care functions.
  • Assist residents with nail care.
  • Shave male residents.
  • Keep hair on female residents clean as instructed.
  • Assist residents with bowel and bladder functions.
  • Maintain intake and output records as instructed.
  • Keep incontinent residents clean and dry.
  • Check and report bowel movements and character of stool as instructed.
  • Utilize protective and adaptive equipment as instructed.
  • Provide eye and ear care as instructed.
  • Weigh and measure residents as instructed.
  • Measure and record temperatures, pulse, and respirations (TPRs) as instructed.
  • Assist with the care of end-of-life residents.
  • Assist with post-mortem care as instructed.
