About The Position

Mosaic is seeking a Mosaic at Home Coordinator/Case Manager to support people with intellectual and developmental disabilities in Pottawattamie County. The role builds relationships with supported individuals, families, host home providers, and direct care workers, and helps ensure individuals’ goals, needs, and preferences are heard and respected. The coordinator collaborates with the service planning team to develop Individual Support Plans that reflect each person’s wishes while meeting applicable policies and regulations. The position also includes monitoring provider relationships and service delivery, conducting regular home and service environment visits, checking contract compliance, and providing on-call assistance to providers and direct care workers, including crisis intervention. The role is hybrid, with travel for home visits and work at the Denison office; travel may be up to two hours. An agency vehicle or mileage reimbursement is available.

Requirements

• Minimum of two years of related work experience.
• Valid driver's license in good standing.
• Strong communication and relationship-building skills.
• Excellent problem-solving abilities.
• Computer proficiency.
• Experience in behavior management.
• Understanding of HCBS industry regulations.

Nice To Haves

• Bachelor’s degree in human services or a related field.
• Previous experience working with people with intellectual disabilities.

Responsibilities

• Develop relationships with supported individuals, their families, host home providers, and direct care workers.
• Help individuals communicate their goals, dreams, and needs, and ensure their voices are respected.
• Monitor relationships between individuals and home providers to support positive, appropriate matches.
• Collaborate with the service planning team to create Individual Support Plans that incorporate individuals’ wishes and meet policy and regulatory requirements.
• Oversee service delivery and contract compliance through regular home and service environment visits.
• Provide on-call support and problem-solving assistance to providers and direct care workers, including crisis intervention.
