About The Position

Mosaic is hiring a Mosaic at Home Coordinator/Case Manager to support people with intellectual and developmental disabilities in Pottawattamie County. The role builds relationships with individuals, families, host home providers, and direct care workers, and helps ensure that each person’s goals, needs, and preferences shape their support. The Coordinator collaborates on Individual Support Plans, monitors provider-individual relationships, oversees service delivery and contract compliance, and conducts regular visits to homes and service environments. The position also provides on-call support to providers and direct care workers, including crisis intervention and problem-solving. The role is hybrid and involves travel for home visits, which may be up to two hours; an agency vehicle or mileage reimbursement is available.

Requirements

• Minimum of two years of related work experience.
• Valid driver's license in good standing.
• Strong communication and relationship-building skills.
• Excellent problem-solving abilities.
• Computer proficiency.
• Experience in behavior management and knowledge of HCBS industry regulations.

Nice To Haves

• Bachelor’s degree in human services or a related field.
• Previous experience working with people with intellectual disabilities.

Responsibilities

• Develop relationships with individuals receiving support, their families, host home providers, and direct care workers.
• Help individuals express their goals, dreams, and needs, and ensure their voices are respected.
• Monitor relationships between individuals and home providers to support positive and effective matches.
• Collaborate with the service planning team to create Individual Support Plans that reflect individual preferences and meet policy and regulatory requirements.
• Conduct regular home and service environment visits.
• Oversee service delivery and contract compliance.
• Provide on-call support, including crisis intervention and problem-solving, to providers and direct care workers.
