About The Position

Mosaic is hiring a Mosaic at Home Coordinator/Case Manager to support people with intellectual and developmental disabilities in Pottawattamie County. This hybrid role develops relationships with individuals served, their families, host home providers, and direct care workers, and helps ensure each person has a voice and receives personalized support. The Coordinator works with the service planning team to create Individual Support Plans that reflect individuals’ goals and needs while meeting applicable policies and regulations. The role includes monitoring provider relationships, overseeing service delivery and contract compliance through regular home and service-environment visits, and providing on-call support for providers and direct care workers. The position is salaried, involves daytime work with on-call crisis intervention, and requires travel for home visits, which may take up to two hours. An agency vehicle or mileage reimbursement is available.

Requirements

• At least two years of related work experience.
• Valid driver's license in good standing.
• Strong communication and relationship-building skills.
• Excellent problem-solving abilities.
• Computer proficiency.
• Experience in behavior management.
• Working knowledge of HCBS industry regulations.

Nice To Haves

• Bachelor’s degree in human services or a related field is preferred.
• Previous experience working with people with intellectual disabilities.

Responsibilities

• Build relationships with individuals served, their families, host home providers, and direct care workers.
• Help individuals express their goals, dreams, and needs and ensure their voices are respected.
• Monitor relationships between individuals and home providers to support positive matches.
• Collaborate with the service planning team to develop Individual Support Plans that reflect each person's wishes and meet policy and regulatory requirements.
• Oversee service delivery and contract compliance through regular home and service-environment visits.
• Provide on-call support to providers and direct care workers, including crisis intervention and problem-solving.
