About The Position

Mosaic is seeking a Mosaic at Home Coordinator/Case Manager to support people with intellectual and developmental disabilities in Pottawattamie County. The role builds relationships with individuals served, their families, host home providers, and direct care workers, and helps ensure each person’s goals and needs are heard and respected. The coordinator monitors relationships between individuals and home providers, collaborates with the service planning team on Individual Support Plans, and oversees service delivery and contract compliance through regular home and service-environment visits. The position also provides on-call support to providers and direct care workers, including crisis intervention. The role is hybrid, involves travel for home visits, and is associated with an office in Denison; travel may be up to two hours. An agency vehicle or mileage reimbursement is available.

Requirements

• A minimum of two years of related work experience.
• A valid driver's license in good standing.
• Strong communication and relationship-building skills.
• Excellent problem-solving abilities.
• Computer proficiency.
• Experience in behavior management.
• Knowledge of HCBS industry regulations.

Nice To Haves

• Bachelor’s degree in human services or a related field.
• Previous experience working with people with intellectual disabilities.

Responsibilities

• Develop relationships with individuals served, their families, host home providers, and direct care workers.
• Help individuals express their goals, dreams, and needs and ensure their voices are respected.
• Monitor relationships between individuals and home providers to support positive matches.
• Collaborate with the service planning team to create Individual Support Plans that reflect individual preferences and applicable policy and regulatory requirements.
• Oversee service delivery and contract compliance through regular home and service-environment visits.
• Provide on-call support to providers and direct care workers, including crisis intervention and problem-solving.
