The Case Manager coordinates services and benefits for individuals admitted to the crisis stabilization program, builds relationships with clients and community providers, manages data related to contract metrics and program outcomes, and provides direct clinical services, including periodic substance-use assessments, to support crisis stabilization. The role works closely with the Mental Health Team and is primarily attached to BestClub Adult Mental Health Consumer Center, though case management may also be assigned to clients who are not BestClub participants. The Case Manager helps clients access resources and benefits, pursue treatment-plan goals, coordinate behavioral, primary, and dental care, and engage with community services. The position may include transportation, home visits and welfare checks, follow-up after hospitalization, group activities, clinical documentation and billing, and—after appropriate training—Social Security payee services. The role also participates in training and helps orient and train newer staff. The position requires a bachelor's degree in a behavioral science field, or an associate's degree in behavioral science plus four years of demonstrated social-services or counseling work. Experience with individuals with severe and persistent mental illness is required. Individuals in recovery from substance-use disorder must have maintained continuous recovery for the two years preceding hire. A QMHA-I certificate is required, or the candidate must be eligible to register for QMHA-R within one week of hire and obtain certification within six months. A valid Oregon driver's license, or ability to obtain one upon hire, and eligibility for the organization's auto liability coverage are also required. English/Spanish bilingual ability is preferred.
