About The Position

Case Managers have significant independence in managing their caseloads, making it suitable for individuals who prefer self-led group work, consultation, collaboration, and then independent task completion.

Requirements

  • Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field or Be a graduate from an accredited four-year college with a major in any field
  • One year of full-time experience as a social worker/case manager/care coordinator in a public or private social service agency.
  • One year of experience in education or treatment of persons served with developmental disabilities or related conditions
  • At least one course focused on developmental disabilities is required. Candidates must provide an official college or university transcript to verify completion of this coursework.
  • Applicants must have a valid driver’s license and acceptable driving record as determined by Pinnacle Services policy.
  • Applicants must have a vehicle that is covered with current insurance.
  • Applicants must successfully clear local and/or stated background checks.
  • Applicants must be willing to commute to areas of Hennepin and Scott Counties for potential meetings.

Nice To Haves

  • One year of full-time experience in education or treatment of people with developmental disabilities or related conditions, or a minimum of one course that specifically focuses on developmental disabilities.
  • Possess a LSW or higher form of BOSW Licensure.

Responsibilities

  • Driving to attend client meetings in Scott County, with an average of 4 client meetings per week.
  • Assessing the areas of the individual's life where assistance is needed.
  • Offering and coordinating resources for the individuals being supported.
  • Monitoring service delivery and working with providers to ensure the individual's needs are met.
  • Service plan development
  • Attend regular in-person meetings with individuals served and their interdisciplinary teams.
  • Case Noting client-related work to create Billable Units
