About The Position
Facilitate the care of clients requiring follow-up services for abnormal breast and cervical results in accordance to program and MDHHS guidelines.
Requirements
- Licensed to practice as a Licensed Practical Nurse (LPN) by the state of Michigan
- Familiarity with chart review and data abstraction.
- Computer and word processing skills and the ability to learn new computer applications, including simple databases and graphics.
- Strong communication skills, both verbal and written.
Nice To Haves
- Five years of medical hospital experience preferred.
- Three years of case management experience preferred.
- Oncology and GYN experience preferred.
- Bilingual (Spanish and English) preferred
Responsibilities
- Responsible for follow-up care of all clients in the BCCCP, according to program and MDHHS requirements and guidelines
- Responsible for the collection and entry of abnormal data into MBCIS in a timely manner according to program and MDHHS guidelines.
- Responsible for the collection of all required data through providers, data retrieval systems and faxed reports.
- Work closely with primary care physicians, surgeons, oncologists, labs and MDHHS to ensure timely follow-up.
- Enter all necessary follow-up data into MBCIS in a timely manner according to program and MDHHS guidelines.
