About The Position
Merakey is seeking a Blended Case Manager to join their team in Monaca, PA. This role involves serving as the primary contact for up to 30 individuals, developing and implementing personalized service plans, providing case management, and offering supportive counseling. The position also includes working with county reentry grants to support individuals transitioning from jail back into the community and participating in on-call responsibilities.
Requirements
- High school diploma or GED AND AT LEAST 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science required.
- 2+ years’ experience in public or private human service, with 1 year being direct care required. Experience must be with clients over the age of 18.
- Valid Driver's license required.
- All clearances required by OMHSAS regulations.
