About The Position

Merakey is seeking a Blended Case Manager (BCM) for its program in Philadelphia, Pennsylvania. The BCM develops, coordinates, and implements services for assigned consumers as identified in their service plans. The role supports consumers across behavioral and physical healthcare needs, family functioning and independent living, educational and vocational goals, and socialization.

Requirements

• A bachelor's degree in psychology, social work, nursing, rehabilitation, counseling, or another related human services field.
• A valid driver's license.
• A reliable means of transportation.

Responsibilities

• Develop services for assigned consumers as identified in their service plans.
• Coordinate services for assigned consumers.
• Implement services for assigned consumers.
