About The Position

Requirements

• Associate's degree in business or a related field; progressively responsible customer service, travel experience, or related work may substitute year-for-year for college education.
• At least one year of relevant professional work experience in community health or case management.
• Demonstrated knowledge of the Alaska Tribal Health System, including service delivery, travel patterns, and referral systems.
• Knowledge of Alaska Native people and cultures and rural Alaska.
• Knowledge of state, federal, and Tribal healthcare programs.
• Knowledge of state, federal, and public/private insurance, including Medicaid and Medicare.
• Knowledge of customer service concepts and practices.
• Knowledge of basic medical terminology and clinic systems.
• Knowledge of the Privacy Act of 1974 and HIPAA Privacy Rule Act of 1966.
• Skill in patient education and outreach and in working with a clinical care team.
• Ability to use video teleconference equipment or learn the required technical skills.
• Ability to communicate effectively with people in varied professional roles and at varied skill levels, including medical providers, case managers, and health educators.
• Ability to work independently or as a team member.
• Demonstrated ability to assess and prioritize multiple tasks, projects, and demands.
• Demonstrated ability to operate a personal computer using a variety of software applications and platforms.
• Must possess a valid driver's license.
• Must have a clean driving record as defined by ANTHC insurance requirements, specifically a clean three-year driving record.
• Must have reliable transportation.
• Some locations may require driving; the posting also states that an annual release for ANTHC to obtain a current driving record may be required.

Responsibilities

• Collaborate with the Tribal Partner in the region to develop processes and solutions that improve and enhance referral visits to ANMC.
• Serve as the regional liaison to the Consortium and local resource for questions and concerns, connecting directly with ANTHC/ANMC to resolve issues and facilitating communication to appropriate Tribal Partner personnel when requested.
• Coordinate with ANMC providers, case managers, and regional healthcare organizations to ensure appointments, travel arrangements, referrals, lodging, transportation, and other logistics are completed.
• Facilitate interaction and communication with healthcare staff and providers.
• Track patient follow-up and care coordination.
• Communicate with other ANTHC departments to coordinate services as needed.
• Use a tracking system to maintain a database of issues requiring follow-up and ensure appropriate follow-up with customer-owners.
• Confirm insurance eligibility and coordinate with ANTHC Health Benefits Specialists to help patients apply for benefits and keep insurance information current.
• Provide outreach education to patients and ensure patient education materials are distributed in clinics.
• Develop relationships with Tribal Partner personnel and offer educational sessions about Field Office roles and services to encourage their use.
• Proactively support the Tribal Partner when not occupied with other ANTHC responsibilities.
• Train and build relationships with Patient Navigators and other Field Managers, both within and outside the region.
• Proactively connect with ANTHC staff traveling to the region to inform them about issues being resolved or requests.
• Promote educational opportunities that introduce village school students and other residents to potential careers, opportunities, and ANTHC resources.
• Collaborate with the Tribal Partner to resolve issues with Video Teleconference (VTC) appointments between ANMC and patients when requested.
• Maintain appropriate records and prepare required reports.
• Perform other duties as assigned.
