About The Position

Supporting the San Antonio branch as needed. Are you a compassionate hospice leader ready to make a meaningful impact across Central Texas? We are seeking an experienced Hospice Clinical Manager, RN to lead our Austin-based clinical team while providing support to our San Antonio branch when needed. This is an excellent opportunity for a strong hospice professional who thrives in leadership, values patient-centered care, and is passionate about guiding teams through exceptional end-of-life care delivery. As the Hospice Clinical Manager, you will play a vital role in ensuring the delivery of high-quality, compassionate hospice services while mentoring and supporting clinical staff. You will collaborate closely with interdisciplinary teams to ensure every patient and family receives dignified, personalized care during life’s most important moments.

Requirements

  • Current RN License
  • Valid Driver’s License and Auto Insurance
  • Current CPR Certification

Nice To Haves

  • Minimum of 2 years of hospice management experience preferred
  • HomeCare HomeBase experience preferred
  • Strong leadership, communication, and organizational skills
  • Passion for hospice care and supporting patients and families during end-of-life transitions

Responsibilities

  • Lead and oversee hospice clinical operations to ensure quality, continuity, and compliance of patient care services
  • Supervise and support interdisciplinary clinical staff, promoting excellence in patient outcomes and compassionate care
  • Ensure compliance with state and federal hospice regulations, including Conditions of Participation (COPs)
  • Participate in and monitor Quality Assurance and Performance Improvement (QAPI) initiatives
  • Collaborate with the Interdisciplinary Group (IDG) to develop and implement individualized patient care plans
  • Provide education, coaching, and mentorship to clinical team members
  • Promote infection control standards and patient safety practices
  • Identify operational or clinical improvement opportunities and communicate recommendations to leadership
  • Foster a supportive, team-oriented culture focused on service excellence and patient dignity
