About The Position
We are seeking an experienced RN Case Manager to support care coordination within the Emergency Department . This role focuses on discharge planning, utilization review, and ensuring patients receive the appropriate level of care while optimizing healthcare resources.
Requirements
- Active Registered Nurse (RN) license – Massachusetts
- Minimum 3 years of acute care hospital experience
- Experience in case management, discharge planning, or utilization review
- Knowledge of InterQual® or similar utilization review systems
- Strong communication, organizational, and multitasking skills
Nice To Haves
- BSN (Bachelor of Science in Nursing)
- Case Management Certification (CCM or equivalent)
- Prior experience in Emergency Department case management
Responsibilities
- Assess patients for discharge planning and ongoing care needs
- Coordinate care with physicians, nurses, and interdisciplinary teams
- Perform utilization review using tools such as InterQual®
- Ensure appropriate level of care (inpatient vs. observation status)
- Facilitate insurance authorization and reimbursement processes
- Coordinate patient discharges, transfers, and follow-up services
- Advocate for patients and communicate effectively with families
- Identify high-risk patients and connect them with appropriate resources
- Participate in care rounds and maintain accurate documentation
