About The Position

Center of Illinois Home Healthcare, Inc. is hiring an experienced Full-Time Physical Therapist Assistant (PTA) for our Medicare-certified, ACHC-accredited home health agency. We are seeking a compassionate, motivated, and dependable Physical Therapist Assistant with home health experience to provide high-quality rehabilitative care to patients in their homes. The ideal candidate will work closely with the supervising Physical Therapist and interdisciplinary team to help patients improve mobility, restore function, and maximize independence while ensuring compliance with Medicare regulations and agency policies.

Requirements

  • Active Illinois Physical Therapist Assistant (PTA) License required.
  • Graduate of an accredited Physical Therapist Assistant program.
  • Knowledge of Medicare home health regulations and documentation requirements.
  • Experience with electronic medical records (Axxess EMR preferred).
  • Excellent communication, organizational, and time-management skills.
  • Ability to work independently and efficiently in the home health setting.
  • Current CPR certification required.
  • Valid driver's license, reliable transportation, and current auto insurance required.

Nice To Haves

  • Minimum of 1–2 years of home health experience preferred.
  • 1 year of Physical Therapy experience preferred.

Responsibilities

  • Provide skilled physical therapy treatments under the direction of the supervising Physical Therapist.
  • Implement individualized treatment plans established by the Physical Therapist.
  • Monitor and document patient progress, reporting changes in condition promptly.
  • Educate patients and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home safety.
  • Complete accurate, timely, and compliant documentation according to Medicare and agency guidelines.
  • Collaborate with nurses, occupational therapists, speech therapists, home health aides, physicians, and other healthcare professionals to coordinate patient care.
  • Participate in case conferences and interdisciplinary team meetings.
  • Maintain productivity while delivering compassionate, patient-centered care.
  • Follow CMS, ACHC, and agency policies and procedures.
  • Promote quality patient outcomes through effective communication and clinical excellence.
