About The Position

Requirements

• Bachelor’s degree in nursing from an accredited college or university, or an accepted education/experience substitution.
• Current Registered Nurse license from the Maryland State Board of Nursing.
• Provide the facility with a telephone number at which the employee can be reached.
• A valid Maryland motor vehicle operator’s license is required if assigned duties requiring operation of a motor vehicle.

Responsibilities

• Deliver comprehensive, evidence-based nursing services to first-time mothers and their families through individualized home-based care.
• Prioritize services for women experiencing social, economic, and health-related vulnerabilities.
• Support improved maternal and child health, stronger parent-child relationships, and long-term family self-sufficiency.
• Screen for, identify, and refer clients with substance use disorders, particularly opioid use disorder, and co-occurring mental health conditions.
• Connect families to appropriate behavioral health services and collaborate with multidisciplinary teams to coordinate trauma-informed care.
• Educate and support families impacted by Neonatal Abstinence Syndrome, including providing parenting guidance and practical strategies.
• Adhere to the Nurse-Family Partnership model, policies, procedures, Visit-to-Visit Guidelines, and quality benchmarks.
• Partner with community agencies and internal Health Department programs to address complex needs and improve outcomes for high-risk families.
