About The Position

This position is for a Certified Nurse Aide in a Skilled Nursing Facility, working Full-Time during Day shifts. The role involves providing direct resident care, maintaining resident safety and sanitation, participating in care planning, and adhering to facility policies and procedures. The Certified Nurse Aide is expected to demonstrate understanding, patience, and tact in dealing with residents and colleagues, and to maintain confidentiality of resident information. This role requires limited access to the medical record for charting purposes.

Requirements

  • Nursing aide certification is required.
  • If hired without certification, must complete the requirements for nurse aide certification for job retention within 120 days.
  • Heartsaver CPR required within 90 days of hire date.
  • Basic Life Support (BLS) required within 90 days of hire date.
  • Good physical and mental health is required.
  • Manual dexterity is necessary for handling instruments and equipment.
  • Visual and aural acuity are required to make adequate observations of resident's condition.
  • Demonstrates understanding, patience and tact in dealing with residents and fellow workers.
  • Abides by and supports Hospital policies and standards.
  • Trustworthy in safeguarding confidential information.
  • Willingness to work hours that best meet the needs of a specific service.
  • Appearance to be neat and professional at all times.
  • The certified nurse aide is listed as current in the New York State Nurse Aide Registry.
  • The CNA's current nurse aide certification status has been verified with the NYS Nurse Aide Registry.
  • There are no findings or convictions of resident abuse, mistreatment, neglect, or misappropriation of resident property listed on the Registry and the CNA.
  • The printed verification from the Registry verifying all the above is filed in the CNA's personnel records.

Responsibilities

  • Follows work assignments and work schedules.
  • Performs tasks as instructed by the Professional Nurse.
  • Receives and gives nursing reports.
  • Records entries on flow sheets, nutrition sheets, elimination record, activities record, and rehabilitation record.
  • Completes all documentation in electronic form or as instructed.
  • Reports changes in resident's condition to the RN or LPN Charge Nurse.
  • Reports all accidents and incidents observed.
  • Participates in performance assessment and improvement activities.
  • Performs routine resident care including bathing, mouth care, hair care, nail care, hygiene, toileting assistance, feeding, and ambulation assistance.
  • Promotes resident comfort by positioning, lifting, transferring, assisting with ambulation, answering call lights, and assisting with emotional needs.
  • Observes and reports skin breakdowns to prevent decubitus ulcers.
  • Assists with post mortem care.
  • Takes and records weights, temperature, pulse, respiration, and blood pressure.
  • Answers all call bells/lights promptly.
  • Promotes adequate resident nutrition by participating with meal service and feeding residents.
  • Serves and assists residents with in-between snacks and nourishments.
  • Collects and transports specimens.
  • Assists in the care of patients in isolation.
  • Performs prescribed rehabilitation nursing exercises, range of motion, and use of adaptive equipment.
  • Folds and delivers resident's personal clothing.
  • Assists with carrying out the Resident Assessment.
  • Participates in Care Plan conferences.
  • Provides nursing care according to established Care Plan.
  • Recommends changes in the resident's Care Plan and goals.
  • Reviews residents' closet door care plan and informs nurse of any changes in patient condition.
  • Operates all equipment in a safe manner.
  • Reports defective equipment immediately.
  • Is responsible for cleaning resident equipment.
  • Follows policies and procedures related to Standard Precautions.
  • Keeps resident's rooms, dresser drawers, closet, utility rooms, shower/tub rooms, and corridors neat and in order.
  • Follows New York State Requirements for PPD testing.
  • Follows emergency protocols related to Fire/Disaster procedures.
  • Attends continuing educational programs.
  • Participates in DNV and OSHA mandated inservices annually.
  • Promotes and fosters the mission and philosophy of the Facility.
  • Actively supports and participates in Continuous Quality Improvement endeavors.
  • Performs other related duties as required.
  • Maintains confidentiality of all residents care information.
  • Maintains the rights of all residents.
  • Provides privacy for residents.
  • Reports resident's complaints or grievances to Charge Nurse.
