About The Position

This position plans, organizes, and directs quality assurance programs and activities. The role involves developing, implementing, and coordinating QA and assessment programs to enhance resident care. The QA Nurse LVN will attend required facility specific meetings/committees that support the facility QAPI program, monitor trends, and prepare quality reports for the QAPI committee. This role also involves developing processes in conjunction with the QAPI Committee to assess systems of care, identifying educational needs based on findings, eliciting data to monitor care and services, and working collaboratively with the Interdisciplinary team (IDT) to provide education/training. The position also includes tracking, monitoring, and investigating adverse events, overseeing the Facility Infection control program, and participating in the correction of State and Federal survey deficiency corrections. Attendance at facility in-service training programs and maintaining utmost confidentiality are also key aspects of this role.

Requirements

  • RN or Licensed Vocational Nurse (LVN) required
  • 3 years Long Term Care experience
  • Proficient in the Microsoft Suite products.
  • Must maintain all CE for certifications and remain in good standing at all times with the State Board of Licensing
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Nice To Haves

  • Specialized training in QAPI or QAPI Certification a plus.

Responsibilities

  • Plan, organize, and direct quality assurance programs and activities.
  • Develop, implement and coordinate QA and assessment programs to enhance resident care.
  • Attend required facility specific meetings/committees which support the facility QAPI program.
  • Monitor trends and prepare quality reports for the QAPI committee.
  • Develop processes in conjunction with the QAPI Committee to assess systems of care including clinical care, quality of life, and resident choice.
  • Identify need for education based on findings and trends.
  • Elicit data from facility identified systems to monitor care and services with findings compared to benchmarks and targets the faculty has established for performance.
  • Work collaboratively with the Interdisciplinary team (IDT) to provide education/training based on findings.
  • Track, monitor and investigate adverse events with identified action plan.
  • Oversee Facility Infection control program which includes process of identification, reporting, surveillance, and investigation of communicable diseases and infections.
  • Participate in correction of State and Federal survey deficiency corrections as appropriate.
  • Attend facility in service training programs (i.e. HIPAA, Abuse Prevention, Infection Control, etc.).
  • Maintain the utmost level of confidentiality.
  • Report suspected or known breaches.
