LVN PM Shift Full-Time
About The Position
Willow Creek Healthcare Center is hiring a compassionate and skilled Licensed Vocational Nurse (LVN) for a full-time PM-shift role at its skilled nursing facility. The nurse provides attentive, high-quality care while supporting resident safety, comfort, and dignity. The role includes providing direct nursing care in accordance with care plans and physician orders; administering medications and treatments; monitoring and documenting residents’ conditions in the EMR; and communicating concerns to the charge nurse or physician. The LVN also supports admissions, discharges, and transfers, collaborates with CNAs and interdisciplinary team members, and follows infection-control, safety, and confidentiality protocols. The stated pay range is $28.00–$28.50 per hour, plus a shift differential. The employer offers full-time benefits and opportunities for professional growth and continuing education.
Requirements
- Active California LVN license in good standing.
- Current CPR certification.
- Strong communication, organization, and time-management skills.
- Compassionate, dependable, and committed to quality resident care.
Nice To Haves
- Skilled Nursing Facility (SNF) or long-term care experience.
- Experience with EMR systems; PointClickCare is preferred.
Responsibilities
- Provide direct nursing care to residents in accordance with care plans and physician orders.
- Administer medications and treatments accurately and on time.
- Monitor residents for changes in condition and promptly report concerns to the charge nurse or physician.
- Document nursing care, observations, and changes in resident condition in the EMR system.
- Maintain a clean, safe, and supportive environment during overnight hours.
- Assist with admissions, discharges, and transfers as needed.
- Collaborate with CNAs and interdisciplinary team members to deliver resident-centered care.
- Follow infection-control, safety, and confidentiality protocols.
