About The Position

Requirements

• Associate’s Degree in Nursing required
• Current RN license in Wisconsin or through an eNLC compact state
• Valid WI driver’s license and acceptable driving record
• BLS certification (or ability to obtain upon hire)
• Strong communication, problem-solving, and time management skills

Nice To Haves

• Bachelor’s Degree in Nursing preferred
• Home health or hospice experience preferred
• Experience with EPIC preferred

Responsibilities

• Provide hands-on care to patients in their homes—delivering skilled nursing care and making clinical decisions that directly impact a patient’s health and independence.
• Develop, follow, or revise a patient’s plan of care based on ongoing clinical assessments.
• Provide patient education, medication management, and wound care.
• Collaborate closely with case managers, physicians, and interdisciplinary teams.
• Use EPIC to document visits, track progress, and coordinate services.
• Empower patients and families to manage health needs in the home environment.
