About The Position

Dignity Hospice is seeking a dedicated RN Case Manager to join their growing team. This role involves planning, organizing, and directing hospice care for patients in their homes and communities. The RN Case Manager will serve as the central point for the patient's care plan, collaborate with physicians and other clinicians, and mentor Home Health Aides. This is a full-time position with opportunities for professional growth and a focus on quality patient care.

Requirements

Current Texas State license as a Registered Nurse
2+ years’ experience as a Registered Nurse in home health.
Reliable transportation
Current Texas Driver’s License
Valid and current auto liability insurance
Must have Kinnser EMR experience
RN (Required)
Hospice: 2 years (Required)
Kinnser: 2 years (Required)

Nice To Haves

Case management: 1 year (Preferred)
Willingness to travel: 75% (Preferred)

Responsibilities

Performs comprehensive assessment for patient admission
Develops and updates the plan of care in consultation with the physician and all clinicians in charge of patient's care
Participates in the patient's discharge planning process.
Provides skilled nursing care as needed in accordance with the plan of care.
Cooperates with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan.
Always strives to improve nursing care by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations.
Responsible for the overall clinical record maintenance to include visit note documentation be completed within 24 hours, comprehensive assessments, including OASIS, be completed within 48 hours, medication profile updated timely, orders/plans of care submitted timely, etc.
Maintains confidentiality of patients’ private health information.
