About The Position

As an RN Case Manager at Luminos Hospice, you will work with the Clinical Director to lead a care team that includes LPN(s), LVN(s), aides, an MSW, and a chaplain to provide hospice care. The position coordinates and oversees patients’ home care, collaborates with care teams to develop and implement care plans, and educates families about patients’ conditions, needs, and care.

Requirements

Current state license as a Registered Nurse.
Current driver’s license.
Current CPR certification.
Two years of experience as a Registered Nurse in a clinical care setting.
Nursing skills consistent with generally accepted standards of practice.
Knowledge of state and federal regulations.
Ability to work independently and make sound decisions.
Strong attention to detail and organizational habits.
Excellent verbal and written communication skills.
Good interpersonal skills.
Reliable transportation and valid auto liability insurance.

Nice To Haves

One year of full-time hospice experience is preferred.

Responsibilities

Coordinate and oversee all aspects of patients’ home care.
Collaborate with patient care teams to develop and implement care plans.
Educate families about patient conditions, needs, and care.
Perform head-to-toe physical assessments.
Check vital signs and manage medications.
Administer and maintain catheters.
Complete documentation accurately and on time.
Supervise HHAs.
Regularly communicate patient progress to the clinical manager, physician, and care team.
Perform other duties as assigned.
