Requirements

• Active Colorado Nursing License
• Bilingual in English and Spanish
• Strong organizational and communication skills
• A heart for providing comfort, dignity, and exceptional care

Nice To Haves

• 1 year of home health or hospice experience preferred

Responsibilities

• Lead the coordination of compassionate, patient-centered care plans
• Use your Spanish and English communication skills to help ensure patients and families feel heard, understood, and supported
• Build meaningful connections with patients and families during some of life’s most important moments
• Work with autonomy in the field while being supported by an experienced, collaborative team
