About The Position

Requirements

• Licensed by the Commonwealth of MA as a Registered Nurse
• At least two years experience preferred in community health, oncology, hospice, or palliative care
• Excellent communication and interpersonal skills
• Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families
• Highly skilled at providing patient assessments and evaluations
• Excellent problem-solving skills
• Competency with accepted standards of hospice nursing practice

Responsibilities

• provide information about hospice services to patients, families, and hospital-based staff
• conduct clinical assessments
• coordinate hospital discharge plans
• maintain professional relationships within hospitals, with a focus on the assigned hospital
