Physical Therapist (PT) – Home Health Schedule: Mon-Friday 8am-5pm Mercy Medical Homecare Multiple Territory positions available: East Canton, Canton, Canal Fulton, Massillon, East Sparta Job Summary We are seeking a compassionate and dedicated Physical Therapist (PT) to join our Home Health team. The Physical Therapist provides skilled therapy services in patients' homes under physician orders, helping individuals improve mobility, reduce pain, restore function, and maximize independence. The PT develops individualized plans of care, educates patients and caregivers, and collaborates with the interdisciplinary team to achieve exceptional patient outcomes while maintaining compliance with Medicare, state, and agency guidelines.

Requirements

  • Minimum of one (1) year of experience as a Physical Therapist in an acute care setting required.
  • Current, unrestricted PT license in the state of employment.
  • Strong clinical assessment, critical thinking, and patient education skills.
  • Excellent communication and collaboration skills with patients, families, physicians, and healthcare professionals.
  • Ability to prioritize workload, meet documentation deadlines, and work independently.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Graduate of an accredited Physical Therapy program.
  • Current license to practice as a Physical Therapist (PT) in the state of employment.

Nice To Haves

  • Home health experience preferred.

Responsibilities

  • Provide skilled home health physical therapy services as ordered by the attending physician.
  • Perform comprehensive evaluations, including OASIS assessments, and develop individualized plans of care.
  • As the treating Physical Therapist, assess patient progress and modify treatment plans based on clinical findings.
  • Deliver evidence-based interventions to improve strength, balance, mobility, endurance, and functional independence.
  • Educate patients and caregivers on disease management, fall prevention, home exercise programs, and the safe use of assistive devices.
  • Communicate patient status, progress, and recommendations to physicians and the interdisciplinary care team.
  • Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and agency policies.
  • Ensure continuity of care through admissions, recertifications, resumptions of care, transfers, and discharges.
  • Complete all required clinical documentation accurately and within agency timeframes, including Start of Care, evaluation, recertification, routine visit, and discharge documentation.
  • Maintain compliance with Medicare, federal, state, and company documentation standards.
  • Participate in case conferences, on-call responsibilities, mandatory meetings, and continuing education.
  • Travel to patient homes according to the daily visit schedule.
  • Perform other duties as assigned.
