About The Position
Human Touch Home Health is hiring Physical Therapists (PTs) for Full-Time, Part-Time, and PRN home health opportunities in Germantown, Damascus, Cabin John, and surrounding Montgomery County communities. We're looking for therapists who want meaningful one-on-one patient care with the flexibility to choose a territory that works for them. You do NOT have to cover all of Montgomery County.
Requirements
- Active Maryland Physical Therapist License Required
- Graduate of an accredited Physical Therapy program
- Reliable transportation
- Strong clinical and communication skills
- Ability to work independently in patients' homes
Nice To Haves
- Home Health Experience Preferred, but Not Required
- WellSky Experience is a Plus
Responsibilities
- Complete physical therapy evaluations in patients' homes
- Develop individualized plans of care
- Help patients improve strength, balance, endurance, and mobility
- Provide therapeutic exercise and functional training
- Provide gait, transfer, and stair training
- Assess fall risks and home safety
- Help patients regain mobility following illness, surgery, injury, or hospitalization
- Educate patients, families, and caregivers
- Monitor patient progress and modify treatment plans
- Complete reassessments and discharge planning
- Collaborate with the interdisciplinary care team
- Complete timely clinical documentation using WellSky
