About The Position

Human Touch Home Health is seeking Occupational Therapists (OTs) for Full-Time, Part-Time, and PRN home health opportunities in Germantown, Clarksburg, Damascus, and surrounding Northern Montgomery County communities. This role offers the chance to focus on patients within Northern Montgomery County, reducing driving time and increasing patient interaction. OTs will have the flexibility to choose their schedule and preferred territory within the designated area, ensuring consistent patient opportunities and mileage reimbursement. The position emphasizes one-on-one patient care, supportive leadership, and opportunities for professional growth. Requirements

Active Maryland Occupational Therapist License Required
Graduate of an accredited Occupational Therapy program
Reliable transportation
Strong clinical, communication, and organizational skills
Ability to work independently in patients' homes

Responsibilities

Complete occupational therapy evaluations in patients' homes
Develop individualized plans of care
Help patients regain independence with activities of daily living
Provide therapeutic interventions to improve functional ability
Address bathing, dressing, grooming, toileting, meal preparation, and household tasks
Evaluate home safety and fall risks
Recommend and train patients on adaptive equipment
Educate patients, families, and caregivers
Monitor patient progress
Complete reassessments and discharge planning
Collaborate with the interdisciplinary care team
Complete timely clinical documentation using WellSky
