Physical Therapist (PT) for Home Health
About The Position
FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Tracy and the surrounding areas. As an independent contractor, you are your own boss and control your own schedule. You will have the flexibility to see patients when and where you want, with no minimum requirement. Compensation is strong per-visit rates, allowing you to determine your earnings by the number of patients you treat. FeldCare Connects provides administrative support, including assigning, communication, scheduling, care coordination, and quality assurance, so you can focus on treating the patient. You will observe, record, and report the patient's response to treatment and changes to the patient's condition, and determine the plan of care. Documentation is made easy with an intuitive Electronic Medical Record system, with many clinicians completing documentation during the visit. You will join a group that shares your passion for helping people and have access to patients through an innovative app, Cliniconnects, to accept or decline patients in seconds.
Requirements
- Current Physical Therapist license issued by the state board of Physical Therapy.
- The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
- Completion of an accredited Physical Therapy program.
- Reliable personal transportation.
- Smartphone.
- Ability and enthusiasm in working with all ages.

Responsibilities
- Deliver quality home healthcare to patients of all ages.
