About The Position

Human Touch Home Health has an immediate need for PRN Physical Therapists (PTs), especially in Alexandria and Arlington, with additional patient opportunities throughout Fairfax County. This role offers flexibility, allowing you to choose the cities you want to cover, share your availability, and build a caseload that works for you. You do NOT have to cover all of Fairfax County or Northern Virginia. The position involves completing physical therapy evaluations in patients' homes, developing individualized plans of care, and providing therapeutic interventions to improve strength, endurance, and functional independence. You will also assess home safety, educate patients and caregivers, monitor progress, and coordinate care with the interdisciplinary team. Accurate and timely documentation using WellSky is required.

Requirements

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical and communication skills
• Ability to work independently in patients' homes

Nice To Haves

• Home Health Experience Preferred, but Not Required

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength, endurance, and functional independence
• Assess home safety and identify fall risks
• Educate patients and caregivers
• Monitor progress and adjust treatment plans as needed
• Complete reassessments and discharge planning
• Coordinate care with the interdisciplinary team
• Complete accurate and timely documentation using WellSky
