About The Position

Requirements

• Associate degree in Nursing; a nursing diploma with RN licensure may be accepted in lieu of the degree.
• One year of experience as a registered nurse, or an accepted in-lieu qualification: successful completion of Prisma Health's Nurse Residency Program or a comparable residency; or, for internal candidates in the same service line, one year of LPN experience.
• Current RN compact/multistate license recognized by the NCSBN Compact State, or a license to practice as an RN in the state where the team member works.

Responsibilities

• Collect patient data and complete nursing assessments using appropriate age- and area-specific standards, policies, tools, and forms.
• Recognize changes in patient status and responses to interventions, establish nursing priorities, and document and validate priorities with the healthcare team.
• Develop, update, and evaluate individualized plans of care in collaboration with patients, families, providers, and the interdisciplinary healthcare team.
• Establish measurable patient goals and outcomes, plan for discharge and continuity of care, and arrange appropriate referrals.
• Provide direct patient care consistent with the plan of care, applicable policies and procedures, and the RN scope of practice.
• Delegate appropriate nursing tasks, provide assignments when appropriate, and ensure assigned tasks are completed.
• Monitor the quality of care and patient safety, coordinate care and resources, and incorporate cost-containment principles.
• Use appropriate technology and complete assigned hospital or unit performance-improvement monitoring and audits.
• Evaluate patient responses and progress toward outcomes, revise plans of care as needed, and report changes or unresolved problems to appropriate team members.
• Communicate patient outcomes and pertinent care information with peers and other healthcare team members.
• Identify patient and family learning needs, provide and evaluate education, and communicate education needs across the healthcare continuum.
• Evaluate personal nursing practice, maintain competency and professional licensure, and complete required education and training.
• Serve as patient advocate and care coordinator, collaborating with providers and other members of the healthcare team.
• Perform other duties as assigned.
