The company is currently expanding in the Cleveland area and is seeking a Registered Nurse (RN) Case Manager to join their team.

Requirements

  • Registered Nurse (RN) licensure for the state of Ohio
  • Clean driving record
  • Valid driver’s license
  • Auto insurance

Responsibilities

  • Provide comprehensive care coordination and support to hospice patients and their families.
  • Assess patient needs.
  • Develop individualized care plans.
  • Coordinate hospice care services.
  • Provide emotional support throughout the end-of-life journey.
  • Clinical Assessment and Care Planning
  • Symptom Management and Pain Control
  • Emotional Support and Counseling
  • Care Coordination and Collaboration
  • Documentation and Compliance
  • On-call Rotation for after-hours and weekends
