  • Registered Nurse

Responsibilities

  • Assume responsibility for coordination of the hospital-wide Diabetes Management Program.
  • Develop, deliver, evaluate and conduct diabetes management activities and education to hospital staff, patients and family members including diabetes medications, insulin pumps, continuous glucose monitors and other education needs.
  • Lead an outcome-based approach to patient care by insuring evidence-based practice and interdisciplinary care delivery.
  • Identify safety concerns, and collaborate with all members of the healthcare team to institute measures to ensure a safe and timely discharge.
