The Licensed Professional Clinical Counselor provides therapeutic health services, counseling and case management for the patients including high risk and low-income individuals and families. The LPCC will be working as part of our multidisciplinary team with an emphasis on our chronic patients, with conditions such as diabetes, cardiovascular disease, substance abuse and depression. May include traveling to our various sites.

Requirements

  • Master's in psychology, counseling or other related field from an accredited college or university.
  • Must be licensed as a Licensed Professional Clinical Counselor in the state of California.
  • Organized, self-driven, team player with technical skills.
  • An understanding of the cultural sensitivities in this type of work and the use of cultural appropriate methods is important.
  • Hearing: Sufficient to engage with patients and staff in person and by telephone.
  • Speaking: Clear verbal communication to convey medical and operational information.
  • Vision: Visual acuity adequate for reading printed materials and viewing electronic screens.
  • Cognitive Abilities: Aptitude to complete tasks, including abilities such as learning, remembering, focusing, categorizing, and integrating information for decision-making, problem-solving, and comprehending.
  • Motor Function & Mobility: Ability to move within the work environment to complete tasks and interact with others.
  • Occasional lifting and carrying of items up to 10 pounds without assistance.
  • Frequent standing, sitting, bending, reaching, and other movements typical of a standard workday.
  • Regular use of hands and fingers for typing, writing, operating tools or equipment, and handling materials.
  • Some pushing and pulling required during daily tasks.

Nice To Haves

  • Spanish speaking is preferred but not required.
  • Experience working with farm worker communities and under-served populations is a plus.

Responsibilities

  • Participating in interdisciplinary treatment planning
  • Evaluating data to identify causes of problem of individuals and to determine advisability of counseling or mayсці refer to other specialist or institutions
  • Counseling or therapeutic interviewing to assist individual to gain insight into personal problems, define goals, and plan action reflecting interest, abilities and need
  • Following up on results of counseling to determine reliability and validity of treatment used
  • Documenting in medical record according to requirements of company and regulatory agencies
  • Providing crisis intervention consultation
  • Providing consultation/education to community service clubs, organizations, human services agencies, hospitals, and various county departments
  • Program development and staff in-services
  • Performing administrative functions consistent with the needs of the program
  • Participating in staff meetings as required
  • Meet monthly productivity requirement and assigned goals
  • Provide the best customer care possible, and the ability to be reliable working a flexible schedule as needed
  • Identify and build upon the strengths of consumers, coworkers, and the communities we serve
  • Support consumers’ steps towards recovery and wellness
  • Create an organizational culture that respects and celebrates the diversity of our consumers
  • Value learning as an ongoing process that enables us to better serve our communities
  • Research and utilize our industry’s best practices and analyze our own services to ensure the best possible outcomes
