About The Position

The Clinical Pharmacist will be responsible for ensuring that the practice of pharmacy is in overall compliance with laws and regulations. This role provides professional pharmacy services as appropriate, assists in the development of clinical pharmacy goals and objectives, and maintains up-to-date patient drug profiles. The pharmacist will provide patient medication counseling and education, implement clinical pharmacy activities with an emphasis on therapeutic drug monitoring, clinical pathways, formulary management, pharmacoeconomics, and patient outcomes. They will also perform Medicare Annual Wellness Visits, collaborate with physicians on medication use criteria, and respond to drug information requests. Additionally, the role involves administering immunizations, monitoring specific drugs, participating in Medication Therapy Management, analyzing scientific literature, educating healthcare professionals and patients, interpreting pharmacokinetic data, establishing cost-effective drug therapy programs, and utilizing computer applications for work assignments. The pharmacist must adhere to CVHS policies and procedures, access protected health information appropriately, and follow accreditation standards for Mail Order Pharmacy and Specialty Pharmacy.

Requirements

  • Graduation from an accredited Pharmacy School with a Bachelor of Science in Pharmacy or PharmD.
  • Licensed to practice pharmacy in the Commonwealth of Virginia.
  • License is free of restrictions to bill Medicaid and Medicare.
  • Completion of pharmacy-based immunization certificate or equivalent.
  • Basic Life Support CPR certification.

Responsibilities

  • Ensuring that the practice of pharmacy is in overall compliance with laws and regulations.
  • Providing professional pharmacy services as appropriate.
  • Assisting in development of appropriate clinical pharmacy goals and objectives.
  • Maintaining and using up-to-date patient drug profiles.
  • Providing patient medication counseling and education through the proper use of labels, accessory labels and personal contact.
  • Implementing and providing all aspects of clinical pharmacy activities with emphasis on therapeutic drug monitoring, clinical pathways, formulary management, Pharmacoeconomics and patient outcomes through detailed reporting systems.
  • Implementing policies, practices and procedures for clinical pharmacy services, ensuring compliance with regulatory standards.
  • Performing Medicare Annual Wellness Visits which will include, but is not limited to, medication reconciliation, laboratory reviews, medication appropriateness, immunizations required, preventative health screenings required, and address any questions that the patient may have about their care.
  • Collaborating with physicians in establishing drug criteria for the safe, economical and appropriate use of medications.
  • Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and healthcare providers including therapeutic duplications, medication allergies, clinically significant drug-drug, drug-disease, drug-nutrient, or drug-laboratory test interactions.
  • Administer immunizations based on collaborative practice agreement or standing orders.
  • Performing initial assessments of drugs having pre-established criteria to be monitored and monitors these drugs for follow-ups.
  • Participate in Medication Therapy Management and Comprehensive Medication Reviews as applicable.
  • Retrieving, analyzing, evaluating, and interpreting the scientific literature, and serving as the resource person for patient and population-specific drug information to health professionals and patients.
  • Participating in the generation of new knowledge relevant to the practice of pharmacotherapy, clinical pharmacy and medicine.
  • Educating health care professionals, pharmacy students, patients, and the public regarding rational drug therapy.
  • Interpreting and applying pharmacokinetic drug data, and recommending dosing regimens based on patient's medical history and disease state, ensuring optimal patient care.
  • Establishing and implementing programs that help ensure cost-effective drug therapy and successful patient outcomes.
  • Attending conferences, seminars, and meetings as required.
  • Using computer applications or other automated systems such as spreadsheets, word processing, calendar, e-mail and database software in performing work assignments.
  • Being familiar with and following the policy and procedures of CVHS and CVHS Pharmacy.
  • Accessing protected health information (PHI) in accordance with departmental assignments and guidelines.
  • Abiding by the standards of accreditation in Mail Order Pharmacy and Specialty Pharmacy.
  • Performing other incidental tasks, as needed.
  • Attending work as scheduled.
