About The Position

Work Schedule: 100% FTE, evening shifts varying between 12:30 PM – 10:00 PM. 1 in 4 weekends and holiday rotation required. Hours may vary based on the operational needs of the department. We are seeking a Clinical Pharmacist – Critical Care Provide medication therapy management services as part of the interdisciplinary team to patients on inpatient services to assure that drug therapy is safe, effective, efficient, and cost-effective. Actively participate in bedside medical emergencies, providing expertise in medication selection, preparation, procurement, documentation, and medication history review. Engage in interdisciplinary patient care rounds, including medication reconciliation, therapy monitoring, order verification, patient education, drug information support, and individualized drug dosing. Provide patient care to a diverse population including critical care, medical and surgical patients. Teach and precept Pharmacy Residents and PharmD students through structured practical experiences and formal didactic education.

Requirements

  • Bachelor's Degree in Pharmacy or a PharmD degree
  • 1 year of clinical patient care and orders management experience
  • Licensed to practice pharmacy in the State of Wisconsin within 90 Days
  • Basic Life Support (BLS) within 90 Days
  • Advanced Cardio Life Support (ACLS) within 180 Days
  • Pediatric Advanced Life Support (PALS) within 180 Days

Nice To Haves

  • Completion of an ASHP Accredited Residency training program
  • 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program
  • Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.)
