Virtual Clinician Home Evaluations - Nurse Practitioner or Physician Assistant (Full Time) - MN
About The Position
Signify Health, part of CVS Health, is hiring full-time, board-certified nurse practitioners or physician assistants to conduct online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members. Clinicians conduct approximately 30-minute visits that include reviewing medication and medical history, educating members about their health, and answering questions. Clinicians are dedicated to telehealth services only and help members connect with care suited to their individual needs. The role requires 40 hours of availability each week, with a target of at least 320 capacity units and 220 completed visits per month. Clinicians must complete required training, obtain credentials for applicable health plan clients and lines of business in their primary coverage states, maintain relevant licenses and board certification, attend regular manager, team, and clinical meetings, and meet operational, clinical, and member-experience metrics. Visits must be conducted from a private, quiet setting with adequate broadband, enabled video and audio, a neutral background, and protections for member privacy. The position requires autonomous work and effective communication and technology skills. The role offers a base salary plus an incentive structure based on the number of in-home visits completed, along with malpractice insurance coverage during Signify Health-related activities. Requirements
- Board-certified nurse practitioner or physician assistant.
- Maintain active state licenses in coverage areas.
- Ability to obtain and maintain multistate licenses in coverage areas, as required.
- Maintain board certification.
- Meet minimum requirements for remote visit delivery, including broadband connectivity with video and audio enabled, and a quiet setting with a neutral background.
- Ability to uphold health plan member privacy per CVS Health guidelines.
- Effective verbal, written, and electronic communication skills.
- Ability to work without direct supervision and practice autonomously.
- Proficiency with information management and technology.
- Ability to interact remotely with health plan members, sit, write, operate a computer and phone, speak intelligibly, and hear member responses.
- Ability to view and read a computer screen, iPad, and other electronic devices.
Nice To Haves
- For nurse practitioners, FNP or AGNP certification is preferred.
- Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format.
Responsibilities
- Conduct online Video Visits with health plan members in a private space with adequate internet connectivity.
- Review members’ medication and medical histories during approximately 30-minute visits, educate them about their health, and answer questions.
- Help connect members to appropriate care for their individual needs.
- Provide 40 hours of availability weekly, consistently delivering at least 320 capacity units per month.
- Maintain a strong visit completion rate, consistently pacing at least 220 completed visits per month.
- Complete regular training and maintain affiliation and compliance with Video Visit requirements.
- Obtain training and credentials for health plan clients and lines of business in primary coverage state(s).
- Participate consistently in manager, team, and clinical meetings.
- Meet Signify Health operational, clinical, and member-experience performance indicators and metrics.
- Maintain active state licenses in coverage areas and board certification.
- Uphold health plan member privacy and remote-visit requirements.
